The problem is almost always in the muscles that attach to it.

What's Actually Causing Your IT Band Pain
The iliotibial band is a thick strip of fascia running from the hip to just below the knee on the outer leg. It doesn't contract — it transmits tension from the muscles above it. When the tensor fasciae latae at the hip and the vastus lateralis of the outer quadriceps become overloaded and develop adhesions, they pull the IT band into a state of chronic tension that creates friction against the lateral femoral condyle with every repetitive knee flexion cycle. The result is the burning, sharp lateral knee pain that typically comes on after a predictable distance and forces you to stop.
Foam rolling the IT band directly provides temporary relief by desensitizing the area — but it doesn't address the muscle tension generating the problem. Clinical soft-tissue work targets the TFL, gluteus medius, and outer quadriceps directly, releasing the structures that are actually driving your symptoms.
The gluteus medius is particularly important: when it's weak or inhibited, the TFL overworks to compensate during every stride. Addressing this muscle is often the key to long-term resolution rather than ongoing management.
Treatment That Gets to the Source
Your session begins with a brief intake — your training volume, when symptoms appear, how long you've had them, and what you've already tried. This helps identify whether the primary driver is TFL tension, glute medius inhibition, or both.
Treatment addresses the TFL and hip flexor group, gluteus medius and minimus, outer quadriceps, and the IT band attachment points at the hip and knee. Most clients notice a significant reduction in lateral tension within the first session. Full resolution of chronic IT band syndrome typically requires two to four sessions alongside modifications to training load.
